When you apply for your Pension Benefits, please include copies of the following:

1. Your birth or baptismal certificate.
2. A birth or baptismal certificate for your spouse, if you are married.
3. A copy of your marriage certificate, if you are married.
4. Death certificate or divorce decree concerning any previous marriage, if applicable.
